help us help you by becoming familiar with your insurance plans for the year 2019.
Does your insurance plan include a co-pay?
Before coming in for your office visit, please check your insurance plan to determine if you have a co-pay.
The co-pay amount should be present on your insurance card. If your carrier and plan includes a co-pay,
you will be prompted to provide this to the front desk on the day of your office visit. If the front desk staff
have not prompted you for your co-pay, feel free to remind them about the details of your insurance plan
for future reference in order to avoid repaying missed co-pays.
Does your insurance plan include co-insurance?
Before coming in for your procedure, please check your insurance plan to determine if you have
co-insurance. If you have co-insurance, you will be expected to pay the agreed-upon percentage towards
your procedure that is reinforced by your insurance policy.
Does your insurance plan include a deductible?
Before coming in for your office visit or procedure, please check your insurance plan to determine if you
have a deductible. If you have a deductible, you must pay the full balance of your deductible before your
insurance will activate to aid you in receiving your care. If the front desk staff have not prompted you
following your office visit or procedure, please remind them of your insurance details.
Do you believe you have missed payments in the past?
If you believe you have not paid all of your costs determined by your insurance plan, please feel free to
approach the front desk or call prior to your next appointment to set up a payment plan.
